Simple File Uploader is a simple PHP script to allow you to upload a file to a folder on your website.

Before uploading you are prompted to enter a password. This is an alternative to FTP and features a folder listing script within each upload directory.

Simple File Uploader doesn't require a database and is very simple to amend to suit your particular web server environment.
